Dear WIS Parents,
We hope you are having a great year! Here are a number of documents for your information and completion. Please click on the document title for more information.
1. Medical Form: This will assist us in knowing about any health issues your child is facing. All students entering Kindergarten, 6th grade and students new to WIS need to fill out the Minimum Immunization Requirements Form (page 4). All other students may fill out the Minimum Immunization Requirements Form only if there have been changes in your child's immunization history.
All remaining medical sheets must be completed for all students. All medical forms must be completed and returned to the school within the first month of enrollment.
2. Dress Code: Please refer to this document to familiarize yourself with the school's dress code as you shop for your child's school clothes. Dress code pertains to students in kindergarten to grade 8.
3. Before and After School Care: Sign up your child for before and after school care with this form. We will bill you monthly for your child's participation in this program.
4. Emergency Contact Form: Please complete this form for your child so that we may update our records. This should be turned in by the first day of school.
